Our MissionTo facilitate the participation of people with disabilities in the life of the community and its economy. This mission is addressed in the context of both advocacy activities and service provision.

Family Support

Provides supports and services for families who have a son or daughter with an intellectual disability living in Prince Edward County. Ages range from birth through to and including adults.
 
Supports and Services include:
  • Assisting with the development of a Person Directed Plan                                                 
  • Assisting with long term planning
  • Providing service coordination and connecting with other community services
  • Support with advocacy and facilitating inclusion
  • Coordinating and/or attending case conferences
  • Assist with the transition to high school
  • Assist families to apply for and utilize Ministry funding and develop respite plans
  • Brokering Ministry funds on behalf of the family
  • Organizing and facilitating various workshops, support groups, etc.
  • Assisting families to plan for and make summer connections, community connections, plan for life after school, etc.
  • Assist families to access a range of developmental services for their son or daughter by completing the initial point of contact and referral packages.
Who can make a referral:
  • a parent or adult with an intellectual disability can make a self-referral
  • anyone can make a referral with parental consent
  • referrals can be made over the phone, via email or by completing a referral form
