Planning
 
Planning is the natural process of making decisions about our future, which provides opportunity to celebrate accomplishments and set and prioritize goals.
  • Community Living Prince Edward believes and supports that people direct all aspects of their plans, and recognizes that each person's plan is as diverse as the person themselves. People are supported to self-determine how they prefer to plan, who will participate and what information will be discussed and shared.
  • Plans identify people's vision, goals, desires and the support required to achieve desired outcomes in their life. A variety of planning formats are utilized, which include Person Directed Planning, Transition from School to Community Life Planning, Family Directed Planning and Child Care Planning.
  • Planning happens both formally and informally with people. People are supported to host formal planning meetings every 12-18 months, or more or less frequently based on their preference. Planning is not, however, an event or a one-time occurrence in a person's life, but is an ongoing process that remains fluid in response to each person's changing needs and desires throughout their life.
  • A person's Plan is a living document which is regularly reviewed, updated and modified based on people's changing needs, interests, assessments and as goals are attained. Strategies that support the goals are implemented on a daily basis, and supports are revised as needed.
