Community Homes

Community Living Prince Edward provides quality supports and services for adults living in a number of different Community settings. This includes a transition home for young adults providing specialized supports and services geared toward developing more independent living skills.
Person Directed Plans ensure that supports are provided based on individual perference, interests, and needs.  Each person directs the support they receive. Supports vary from person to person but may include things such as:
  • assistance with daily life skills
  • personal care
  • financial assistance and money management
  • community participation
  • assistance with medical appointments
  • meal planning and shopping
  • connecting with local services and supports.

People are supported to be actively involved in their community and make connections that are meaningful to them

Community Living Prince Edward uses Personal Outcome Measures as a planning tool to ensure that all areas of the person's life are addressed. Staff support each person to pursue their own interests and dreams and work toward their attaining their individualized outcomes.
Community Living Prince Edward recognizes the valuable contributions made by family and friends. Each person is supported to maintain and/or develop a natural support network by fostering communication, contacts, relationships and visits. Family and friends are always welcomed and encouraged to be involved based on individual preference.
